Intake Specialist

Woodhaven is seeking a Intake Specialist who is responsible for fielding calls from potential clients and referral sources, determining eligibility, and providing information and referral services. The Intake Specialist must be able to handle a large volume of telephone calls with a high degree of customer service and often think outside of the box.

The Intake Specialist must always keep the Woodhaven client's best interest top of mind.

Intake Specialist Specific Areas of Responsibility:

  • Works as an extension to the screening process by assisting in answering phone calls, returning messages and utilizing referral sources.
  • Obtains client information by requesting completed applications and medical information; verifying and clarifying information; interviewing clients and family members; explaining admission criteria.
  • Admits clients by completing admission and financial responsibility forms; coordinating and arranging physical, social, emotional, and support service requirements, including transportation.
  • Checks incoming client inventory and checking for contraband.
  • Documenting and filling out incident report for any contraband brought into the facility.
  • Logs client inventory while the client is being processed into the facility
  • Places the client inventory into a dryer for 30 minutes before releasing back to the client.
  • Completes all intake paperwork upon admission. Making sure the client reads and signs all applicable documentation for admission.
  • Performs urinalysis on clients at intake.
  • Places the client into facility clothing (medical scrubs) during the admission process while inventory is taken.
  • Prepares admissions reports by collecting, analyzing, and summarizing data and trends.
  • Keeps clients safe by following safety policies, procedures, and regulations.
  • Protects organization reputation by keeping information confidential.
  • Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks.
  • Enhances organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
  • Performs other duties as assigned and/or requested by Supervisor.


Graduation from an accredited high school or its equivalent.

Three (3) years of experience in an area of specialty or health care setting.

Valid CPR certification at time of hire.
